e2e Declaration Form
If you are an e2e provider and intend to register a learner for EMA:
- If the learner is starting a programme of learning before 31st August 2009 and applying for EMA using a 2008/09 (turquoise) application pack, you MUST click here for the e2e declaration. This includes learners with dates of birth between the 1st September 1989 and the 31st August 1992. The declaration should be downloaded, completed and returned along with the learner’s application for EMA 2008/09. Please note that this form should only be used with a 2008/09 application pack.
- All other learners starting programmes after 31st August must apply on a 2009/10 (green) application pack. Special instructions apply for e2e learners applying on 2009/10 forms, these are detailed in the guidance notes included with the green application pack. Please read these instructions carefully, or the young person may not have their application assessed.

A Guide for Providers Working with Young People
The purpose of this A5 leaflet is to help you advise young people and their families on how EMA might support young people who remain in/return to learning at the end of their compulsory schooling in England. It includes: an explanation of EMA; eligibility criteria; how and when young people should apply for EMA; the EMA Guarantee and information on how to avoid common mistakes when completing an EMA application form.
